Membership Nomination Form

You must be nominated by two faculty members to join The National Society of Leadership and Success. Download a nomination form below. Once you have completed the form, bring it to your chapter advisor (contact us if you do not know your chapter advisor) who will then issue you a code that will allow you to join online.
